Nepalese national applying for following attestations/certifications/Translations are required to submit the supporting documents mentioned therein along with a money order or cashier's check of $ 25.00 as processing fee.

A. Translation of Citizenship Certificate

1. An application (no format);
   
2. A Money Order or Cashier's Check of $ 25.00 drawn in the name of the Embassy of Nepal;
   
3. An original of Citizenship Certificate;
   
4. Copies of applicant's Passport (page 1, 2 and 3);
   
5. Copy of US visa; and
   
6. Postage paid self-addressed return envelope.

3. An original of Driving License;
   
4. Copy of Passport (page 1, 2 and 3); and

2. A Money Order or Cashier's Check of $ 25.00 drawn in the name of the Embassy of Nepal;
   
3. An original of birth certificate;
   
4. Copy of letter issued by Registration Office in Nepal (Registration Section of the Ministry of Local Development or Village Development Committee or Municipality certifying applicant's birth.)
   
5. Postage paid self-addressed return envelope.
   
6. Copy of Citizenship or Passport
   

1. An application with parents' name, applicant's place of birth and date of birth;
   
2. A Money Order or Cashier's Check of $ 25.00 drawn in the name of the Embassy of Nepal;
   
3. Original Citizenship Certificate;
   
4. Copies of Passport (page 1, 2 and 3);
   
5. A copy of US visa;
   
6. If the applicant is unable to submit a copy of Citizenship Certificate, submit a copy of any academic certificate issued by school or college with date of birth mentioned therein; and
   
7. Postage paid self-addressed return envelope.
